On a rainy day six years ago when Ichigo was 9, Ichigo's mother died. Every year the Kurosaki family visit her grave on the anniversary of her death. Alone for a moment at their mother's grave, Yuzu and Karin are attacked by a Hollow known to the Shinigami as Grand Fisher. Rukia saves Yuzu and Karin and keeps Grand Fisher occupied long enough for Ichigo to arrive.
It was Grand Fisher who killed Ichigo's mother, and Ichigo blames himself for not being able to save her. Ichigo and Grand Fisher fight each other until they are both badly wounded. Grand Fisher runs away and Ichigo blames himself for not being able to avenge his mother's death. - by anon
